If you would like to register as a patient at The Limes please see our reception staff. Please bring with you your medical card, you will then be asked to complete two short questionnaires. Please also bring along with you Photo ID preferably your passport or driving licence and proof of address such as a utility bill. Please note: the patient registering must be present with the ID.
Once this has been completed you will be registered onto our computer system and shortly after you will be offered a new patient health assessment. Please bring a specimen of urine along with you for this appointment.

We do have the facility to deal with private patients. Please ask the Managing Partner, Debbie Bodhanya, for details.
When you register with The Limes you register with the practice not an individual practitioner. However you do as a patient have the right to express a preference to see a GP of your choice.
We will endeavour to do our best to offer you an appointment with the GP of your choice but this may not always be possible. Please take into consideration the following points:-

Please print off all the relevant registration forms, fill them out and bring it in with you on your first visit to the practice.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. Please print out this questionnaire, fill it in and bring it with you to the practice.
