If you would like to register as a patient at The Limes please pick up a "new registration" pack from our reception team. If you wish to complete this on site, you will need to bring along with you Photo ID preferably your passport or driving licence and proof of address such as a utility bill. Please note: the patient registering must be present with the ID.
Once this has been completed you will be registered onto our computer system and shortly after you will be offered a new patient health assessment. Please bring a specimen of urine along with you for this appointment.

When you visit the surgery for the first time you will be asked to sign the forms to confirm that the details are correct. Please also bring along with you Photo ID preferably your passport or driving licence and proof of address such as a utility bill. Please note: the patient registering must be present with the ID.
Please print off all the relevant registration forms, fill them out and bring them in with you on your first visit to the practice.

Choosing To See A Practitioner Of Your Choice
When you register with The Limes you register with the practice not an individual practitioner. However you do as a patient have the right to express a preference to see a GP of your choice.
We will endeavor to do our best to offer you an appointment with the GP of your choice but this may not always be possible. Please take into consideration the following points:-
- Some of our doctors only work part-time and therefore may not be working at the surgery when you would like to be seen.
- The GP of your choice may be fully booked on the specific day you would like to be seen.
- The GP of your choice may be on annual leave.
